This simpel little script can be useful to add/sign up personal information about users for example a phonebook, staff list or use it as i did, I used this script so the users of my IRC channel could add information about them self and upload a picture! This script use PHP and All user information is stored in a MySQL table. Its realy easy to use and to modify to fit the user information you need to collect.
